In Diablo 4 Season 15, the gem system continues the restructuring from Lord of Hatred, further reducing the cost of upgrading high-tier gems and increasing the efficiency of gem resource acquisition. Currently, different types of Gems provide different effects in weapon, armor, and jewelry slots, while upgrading Royal, Grand, and Horadric Gems forms the core of endgame gem progression in Season 15.
If your goal is to craft the highest-level Flawless Horadric Gem, the most direct approach is to first reliably acquire Royal Gems and then upgrade them step-by-step according to fixed crafting recipes. For players who want to quickly complete gem progression, Three Ancients is currently a key farming target.
Three Ancients Fast Farming of Royal Gems

In Season 15, efficient Royal Gem farming is based on Three Ancients encounters within Worldstone Keep.
Upon entering the dungeon, there's no need to clear the numerous monsters along the way. Simply follow the walls near the entrance to find the branching paths leading to Ancients area. This area is usually located at the 10 o'clock or 2 o'clock position from the entrance, so quickly moving along one side of the wall will make it relatively easy to find your target.
Once you reach Ancients area, activate the altar and defeat the three Ancients: Madawc, Korlic, and Talic. After completing the encounter, you'll receive a fixed Royal Gem reward, along with other loot.
Here's a crucial farming tip: the fixed Royal Gem reward for Three Ancients does not increase proportionally with increasing torment difficulty. Therefore, if your character cannot comfortably farm T12, there's no need to sacrifice completion speed for a higher difficulty.
A more sensible strategy is to choose a difficulty level where you can quickly defeat Three Ancients, improving overall Royal Gem output efficiency by shortening the time per run. For players who simply want to quickly accumulate Royal Gems, the advantage of speed farming Three Ancients lies in its clear objective, fixed route, and the fact that it doesn't require completely clearing the dungeon.

From Royal Gem to Flawless Horadric Gem
Once Royal Gem is ready, you can proceed with the high-tier gem upgrade process. The entire process is divided into three stages. The first stage is completed at Jeweler, and the latter two stages are completed using Amalgamation in Horadric Cube.
Stage 1: Royal → Grand
At Jeweler, use 10 Royal Gems of the same type to synthesize 1 Grand Gem.
Stage 2: Grand → Horadric
After obtaining te Grand Gem, use Amalgamation in Horadric Cube to synthesize 5 Grand Gems of the same type into 1 Horadric Gem at a 5:1 ratio.
Stage 3: Horadric → Flawless Horadric
Use Amalgamation recipe in Horadric Cube again to synthesize 5 Horadric Gems of the same type into 1 Flawless Horadric Gem.
Therefore, the complete upgrade chain is:
250 Royal → 25 Grand → 5 Horadric → 1 Flawless Horadric
This is also the most important number when planning your Royal Gem Farm. After determining the target gem type, you can calculate backwards how much Royal Gem you need to accumulate, and then arrange your farming plan according to your farming efficiency.
Gems paired with Soul Splinters

Jewelry slots in D4 Season 15 are no longer just for gem selection. With the addition of Soul Splinters, players need to balance the damage or resistance provided by gems with the special effects brought by Soul Splinters.
Soul Splinters are special gems added in Season 15, with 8 types. They provide various special bonuses along with corresponding side effects and can be upgraded in Horadric Cube via Amalgamation.
Unlike gems, the core value of Soul Splinters is not simply increasing damage or resistance, but rather enhancing the build through special effects. For example, Splinter of the Mother allows your character to freely pass through enemy bodies, inflicting vulnerability, and after a certain number of times, you will also be stunned - this effect is particularly useful for builds with high mobility and cleanse mechanics.
Therefore, Season 15 gem setups shouldn't be simply understood as using the highest-level gems in all slots. A more reasonable approach is to use Flawless Horadric Gems for base stats and resistances, and Soul Splinters for the core build mechanics.
With limited Jewelry slots, the number and type of Soul Splinters should be determined based on the build's purpose, and then the remaining slots should be used to supplement the required resistance or other stats. For builds aiming for endgame strength, Flawless Horadric Gems and Soul Splinters are not an either-or system, but rather should be combined as two sets of gems with different functions.
Gem Trading and Resource Considerations
Complete gems can be traded, so if you are missing a certain Diablo 4 Flawless Horadric Gem, you can obtain it through the trading system instead of necessarily farming it from scratch.
For players planning to develop a character long-term, it's recommended to first determine the types of gems needed for your build, and then focus on crafting and upgrading those gems. Especially before crafting high-tier gems, avoid frequently changing your target type due to build adjustments. Also, high-tier gems are not suitable for indiscriminately dismantling for resource recycling. Confirming build requirements before crafting is usually more efficient than salvaging later.
Summary
The development of high-tier gems in Diablo 4 Season 15 can be simply understood as a clear upgrade path:
Three Ancients → Royal Gem → Grand Gem → Horadric Gem → Flawless Horadric Gem
Therefore, to efficiently craft Flawless Horadric Gems, the most important thing is not blindly increasing farming difficulty, but rather determining the target gem type, consistently farming Royal Gems, and planning resources according to the 250:25:5:1 quantity ratio.
